The SCAN Health Plan Community Giving program provides one-time grants for nonprofits and governmental agencies that provide direct services to older adults (age 55+) and caregivers within SCAN Health Plan service areas. Our funding priorities include nutrition/food security, emergency assistance and other projects aligned with the goal of keeping seniors healthy and independent at home.

Our mission is to continue to find innovative ways to enhance seniors' ability to manage their health and to continue to control where and how they live.
